Understanding Keloid Scar Removal Surgery

A keloid is an overgrowth of dense, fibrous scar tissue that develops after the skin has healed from an injury, such as a piercing, surgical incision, burn, or severe acne. Unlike standard scars, keloids grow beyond the original wound boundary, spreading into surrounding healthy skin. They can be itchy, painful, and highly visible, particularly on the earlobes, chest, shoulders, and jawline. Keloids are notoriously difficult to treat because the body's healing response is overactive; simply cutting the scar out often causes it to grow back even larger.

At Shumaila's, our GMC-registered surgeons and dermatologists use a clinically proven combination protocol. We perform precise surgical excision to remove the bulk of the keloid, immediately followed by a course of targeted 5-Fluorouracil (5-FU) and corticosteroid injections. This dual approach suppresses the overactive fibroblasts (scar-producing cells), significantly reducing the risk of recurrence and delivering a flatter, smoother result.

Targeted Injection Therapy (First-Line)

  • For smaller or newer keloids, surgery may not be necessary
  • We use a combination of corticosteroid and 5-Fluorouracil (5-FU) injections directly into the scar tissue
  • This reduces inflammation, breaks down the dense collagen, and gradually flattens the keloid over a series of sessions

Surgical Excision + Injection Protocol (Definitive)

  • The gold standard for large, mature, or highly raised keloids (such as earlobe keloids)
  • The surgeon excises the keloid mass under local anaesthetic, closing the wound with fine sutures
  • Crucially, this is followed by a strict protocol of post-operative steroid/5-FU injections to prevent the scar from returning

Expect results in

Weeks 1–2 (Surgical Healing)

The physical bulk of the keloid is immediately gone. The focus is on allowing the surgical incision to heal cleanly without tension or infection.

Weeks 3–8 (Active Suppression)

During this critical window, you will receive your scheduled steroid/5-FU injections. The new scar will look pink but should remain flat and soft.

Months 3–6 (Fading)

The active suppression phase concludes. The scar tissue stabilises, and the redness begins to fade to a paler tone closer to your natural skin colour.

Months 6–12 (Final Maturation)

The scar fully matures. It remains flat and blends much better with the surrounding skin. Any itching or discomfort from the original keloid is permanently resolved.

Post-Operative Aftercare

Strict adherence to aftercare and follow-up appointments is critical to preventing keloid regrowth. Follow these clinical guidelines:

First 48 Hours: Keep the surgical dressing completely dry and intact. Avoid any strenuous activity that could stretch or pull the incision site.

Wound Care: Follow your surgeon’s specific instructions for cleaning the wound. Do not pick at scabs or apply unapproved creams to the healing incision.

Attend All Injection Appointments: This is the most important step. You MUST attend all scheduled post-operative steroid/5-FU injection sessions to prevent the keloid from returning.

Silicone Therapy: Once the wound has closed, your specialist may recommend applying silicone gel or sheets to provide continuous pressure and hydration to the new scar.

Sun Protection: Keep the healing scar out of direct sunlight for at least 6 months, or use a high-SPF sunscreen, to prevent permanent dark pigmentation (hyperpigmentation).

Frequent Asked Questions

Yes, but surgical excision alone is not enough. Because keloids are caused by an overactive healing response, cutting them out without follow-up treatment often causes them to grow back larger. At Shumaila’s, we combine surgical excision with post-operative steroid and 5-Fluorouracil (5-FU) injections to suppress the scar tissue and prevent regrowth.
