Understanding Lip Reduction Surgery

While lip augmentation is widely discussed, many individuals seek the opposite: a permanent reduction in lip volume to achieve better facial proportion. Lip reduction surgery (clinically known as reduction cheiloplasty) is a highly specialised procedure designed for patients with naturally large lips (macrocheilia), asymmetrical lips, or those seeking to reverse the effects of permanent lip implants or decades of filler use.

At Shumaila's, our GMC-registered surgeons perform this delicate procedure entirely from the inside of the mouth. By removing a precise strip of inner mucosal tissue, we permanently reduce the vermillion volume without leaving any visible external scars. Crucially, we adhere to strict UK clinical safety protocols: if your lip volume is due to recent hyaluronic acid fillers, we will always recommend dissolving the filler with hyaluronidase first, ensuring surgery is only performed when genuinely necessary.

Natural Lip Reduction (Macrocheilia)

  • Designed for patients whose natural lip anatomy feels disproportionately large compared to their other facial features
  • A carefully measured 3–5mm strip of inner mucosal tissue is excised to reduce the overall volume
  • Creates a permanent, balanced reduction in either the upper lip, lower lip, or both simultaneously

Asymmetry Correction

  • Ideal for patients where one lip is significantly larger than the other, or where one side of a lip is uneven
  • The surgeon selectively removes tissue only from the enlarged areas to restore natural symmetry
  • Often combined with subtle reshaping techniques to ensure the upper and lower lips complement each other perfectly

Permanent Enhancement Reversal

  • For patients seeking to reverse the effects of permanent lip implants, silicone injections, or structural changes from years of heavy filler use
  • The surgeon removes the foreign material alongside excess stretched tissue to restore a natural lip contour
  • If recent hyaluronic acid filler is present, we will first use hyaluronidase to dissolve it before assessing if surgery is still required

Safe and Surgically Proven

Lip Reduction Surgery is a well-established procedure when performed by qualified surgeons under UK clinical governance.

At Shumaila’s London, all procedures are delivered by GMC-registered surgeons in CQC-registered environments, with individualised consent and aftercare.

It’s safe because

Performed exclusively by highly experienced, GMC-registered surgeons in our CQC-regulated London and Essex clinics

Carried out under local anaesthetic, completely avoiding the systemic risks and extended recovery associated with general anaesthesia

We enforce a mandatory two-week cooling-off period after consultation, adhering to UK cosmetic surgery best practice for permanent procedures

We prioritise non-surgical reversal (hyaluronidase) first if your lip volume is caused by recent dermal fillers

And effective, due to:

The intraoral approach (incisions made inside the mouth) guarantees absolutely no visible external scarring on the face

The removal of mucosal tissue provides a permanent reduction in volume that will not regenerate or require ongoing maintenance

The use of fine, absorbable sutures ensures secure wound closure while eliminating the need for painful stitch removal

Precise surgical measurement ensures a conservative reduction, preventing the lips from looking unnaturally thin or tight

Expect results in

Weeks 1–2 (Initial Healing)

The primary focus is managing swelling and protecting the internal stitches. The lips will look larger than expected due to surgical trauma. Stick to a soft diet and sleep with your head elevated.

Weeks 3–4 (Suture Dissolution)

The absorbable stitches will dissolve naturally. The lips will start to feel less tight, and the true reduced size will begin to emerge as the major swelling resolves.

Months 2–3 (Tissue Softening)

The internal mucosal tissue continues to remodel. Any firmness or lumps along the incision line inside the mouth will soften and flatten out completely.

Months 3–6 (Final Maturation)

The final, permanent result is achieved. The lips are beautifully proportioned, natural-looking, and completely free of any external surgical scarring.

Year 1+ (Permanent Harmony)

Because the excess tissue was surgically removed, the reduction is permanent. Your lips will age naturally in harmony with the rest of your face.

Post-Operative Aftercare

Proper aftercare is essential to protect the internal incisions and manage swelling. Follow these UK clinical guidelines:

First 72 Hours: Swelling will be at its peak. Apply cold compresses to the outside of the lips for 15 minutes at a time. Sleep with your head elevated on two pillows to encourage fluid drainage.

Dietary Restrictions: Stick to a soft, cool diet for the first 5 to 7 days (e.g., smoothies, soup, mashed vegetables). Avoid hot, spicy, or acidic foods that could irritate the internal incisions. Do not use a straw, as the suction can pull on the stitches.

Oral Hygiene: Maintain excellent oral hygiene to prevent infection. Rinse your mouth gently with an alcohol-free, antiseptic mouthwash after every meal. Brush your teeth very carefully, avoiding the incision lines.

Physical Activity: Avoid strenuous exercise, heavy lifting, and bending over for at least two weeks, as this increases blood pressure and can cause bleeding or increased swelling.

Facial Movement: Minimise excessive talking, wide smiling, or aggressive chewing for the first week to prevent tension on the dissolving sutures.

Frequent Asked Questions

No. The entire procedure is performed through a precise incision made on the inner (wet) surface of the lip, completely hidden inside the mouth. Once healed, there is no visible external scarring whatsoever.
